Medical Mutual Medicare Advantage Plans 2027: MedMutual Advantage Ratings, Plans and Counties

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 9, 2026

KEY TAKEAWAY

Medical Mutual of Ohio sells MedMutual Advantage HMO, HMO-POS and PPO plans in 38 Ohio counties for 2027. One of its two Medicare Advantage contracts earned 4.5 stars in the 2027 ratings, released October 8, 2026, putting it among the higher-rated options in Ohio.

Medical Mutual is the Cleveland-based insurer behind MedMutual Advantage, and it is one of the few Medicare Advantage carriers that sells in only one state. That focus is the point: its plans are built around Ohio networks, and it competes against the national carriers county by county from Cleveland and Toledo to Columbus, Dayton and Cincinnati.

This guide covers the 2027 MedMutual Advantage lineup, the new 2027 star ratings, where the plans are sold, which benefits may be included, and how Medical Mutual compares with the other carriers selling in Ohio. For the full Ohio picture, see our guide to Medicare Advantage plans in Ohio.

Ohio’s Medicare Advantage market is shrinking for 2027, from 212 plans statewide to 180. In that environment, a carrier that is renewing its lineup and holding a 4.5-star contract stands out. Medical Mutual did trim plans in 44 counties for 2026, so check that the specific plan you want is offered in your county.

MedMutual Advantage Star Ratings for 2027

CMS released the 2027 star ratings on October 8, 2026. Medical Mutual of Ohio’s two Medicare Advantage contracts are rated 4.5 stars and 3.5 stars. The 2027 national average is 3.99, and only 15 contracts in the country earned a full 5 stars.

Medical Mutual contract Plans under it 2026 rating
H4497 PPO plans: Select, Preferred, Premium, Elite Preferred 4.5 stars
H6723 HMO and HMO-POS plans: Choice, Plus, Classic and the Elite HMO-POS plans 4 stars

2027 ratings: one contract at 4.5 stars, one at 3.5 stars (CMS, October 8, 2026). Confirm the 2027 rating shown next to your specific plan in Medicare Plan Finder.

Why the 4.5-star contract matters

Contracts rated 4 stars or higher earn quality bonus payments from CMS, which plans generally use to fund extra benefits. The 2027 ratings set those bonuses for 2028, so a 4.5-star contract is a good sign for benefit stability a year from now. A contract below 4 stars does not earn the bonus.

Ratings attach to the contract, not the brand, so two MedMutual Advantage plans in the same county can carry different ratings. The rating shown next to each plan in Medicare Plan Finder is the one that applies to you. See the full 2027 Medicare Advantage star ratings and the top rated Medicare Advantage plans.

MedMutual Advantage plan types 2027: HMO, HMO-POS and PPO plans compared

MedMutual Advantage Plan Types: HMO, HMO-POS and PPO

Medical Mutual sells three plan types. Which one fits depends mostly on how much you value a lower cost against the freedom to see doctors outside the network.

Plan type 2027 MedMutual Advantage plans How it works
HMO Choice, Plus Care from in-network providers, generally with the lowest cost sharing
HMO-POS Classic, Elite Enhanced, Elite Prime, Elite Standard An HMO with point-of-service coverage, so certain benefits are covered out of network
PPO Select, Preferred, Premium, Elite Preferred Out-of-network care covered at a higher cost share, with two out-of-pocket limits

Not every plan is sold in every county, and premiums range from $0 on some plans to a monthly premium on richer PPO options. The Elite plans appear in the 2027 listings with new plan numbers, so if you see one in your county, read its Summary of Benefits closely rather than assuming it matches a plan you had before. For how the structures differ in general, see Medicare Advantage HMO plans and Medicare Advantage PPO plans.

Where Medical Mutual Sells Medicare Advantage in 2027

MedMutual Advantage plans are sold in 38 Ohio counties for 2027, covering the Cleveland, Akron-Canton, Youngstown, Toledo, Columbus, Dayton and Cincinnati areas:

Ashland, Brown, Butler, Clark, Clermont, Columbiana, Cuyahoga, Delaware, Erie, Fairfield, Franklin, Fulton, Geauga, Greene, Hamilton, Hocking, Lake, Licking, Lorain, Lucas, Madison, Mahoning, Medina, Miami, Montgomery, Morrow, Ottawa, Perry, Pickaway, Portage, Sandusky, Stark, Summit, Trumbull, Union, Warren, Wayne and Wood.

The plans available differ by county. A plan sold in Cuyahoga County may not be offered in Hamilton County, and the network behind a plan is built around the hospital systems in that region. If you live outside these 38 counties, Medical Mutual does not sell Medicare Advantage where you are.

Benefits MedMutual Advantage Plans May Include

Every MedMutual Advantage plan covers everything Original Medicare covers and includes Part D drug coverage. Beyond that, plans may include extra benefits such as the following, depending on the plan and your county. Not all benefits are available on all plans.

✔

Dental: $0 preventive dental exams, with comprehensive dental coverage up to $2,500 on select plans.

✔

Vision: a $0 yearly eye exam and an allowance of up to $250 toward glasses or contacts.

✔

Hearing: a $0 yearly hearing exam and low copays for hearing aids.

✔

Travel coverage: up to $7,500 a year when you travel outside Ohio.

✔

Fitness: a SilverSneakers membership.

✔

Other extras on select plans: an over-the-counter allowance, transportation, a 24/7 nurse line and $0 copays for generic drugs.

The travel benefit is the one to notice

Most single-state carriers leave you exposed once you cross the state line. Medical Mutual’s out-of-state travel coverage matters if you winter in Florida or visit family elsewhere. Read exactly what it covers in your plan’s Evidence of Coverage before you count on it.

Medical Mutual vs Other Ohio Medicare Advantage Carriers

In most of its 38 counties, Medical Mutual competes with the national carriers and with Ohio’s regional plans. On 2027 star ratings, here is how the field compares:

Carrier in Ohio 2027 star rating signal
Medical Mutual of Ohio Contracts at 4.5 and 3.5 stars
Humana About 95% of members nationally in 4-star-plus plans, including a 4.5-star Ohio contract
Devoted Health Ohio contracts at 4.5 and 4 stars
Aetna More than 69% of members nationally in 4-star-plus plans
Wellcare (Buckeye) 3.5 stars

Source: CMS 2027 Star Ratings and carrier announcements, October 2026. Ratings vary by contract and county.

Medical Mutual’s edge over the national names is local: an Ohio-built network and a company whose Medicare business lives entirely in the state. The trade-off is reach. If you split time between Ohio and another state for long stretches, a national PPO may still serve you better despite the travel benefit. For a ranked national view, see the best Medicare Advantage plans 2027.

2027 Costs Every MedMutual Advantage Plan Shares

Premiums and copays vary by plan, but these federal 2027 limits apply to every MedMutual Advantage plan with drug coverage:

✔

Part D out-of-pocket cap: $2,400, up from $2,100. Once you reach it, covered drugs cost nothing more that year.

✔

Maximum Part D deductible: $700, up from $615. Many plans set a lower drug deductible.

✔

Highest allowed in-network out-of-pocket limit: $9,850, with $14,800 combined for PPOs. Plans often set lower limits.

✔

Part B premium: you still pay it on top of any plan premium.

Add a plan’s out-of-pocket maximum, the $2,400 drug cap and twelve months of premium to see your realistic worst-case year, and compare that number across plans. Our 2027 costs and benefits by carrier guide walks through it.
