Tufts Medicare Preferred 2027: Massachusetts Medicare Advantage Plans

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 14, 2026

KEY TAKEAWAY

For 2027, Tufts Medicare Preferred sells HMO and PPO plans in 10 Massachusetts counties. The $0 Access PPO is gone, the replacement PPO RX carries a county-based premium, and Tufts’s HMO contract is not among the 15 plans rated 5 stars for 2027. Check the exact plan and rating sold in your ZIP code.

Tufts Medicare Preferred is the Medicare Advantage brand of Point32Health, the company formed when Tufts Health Plan and Harvard Pilgrim Health Care merged. For 2027 it sells Medicare Advantage in Massachusetts, and it is one of the largest regional plans in New England.

If you are shopping Tufts for 2027, there is one thing you need to know before anything else: the $0 premium PPO that many members held was discontinued, and Tufts did not move anyone into the replacement automatically. Below is the current lineup, what the plans cost, where they are sold, and what to check before the Annual Enrollment Period closes on December 7.

Tufts Medicare Advantage Plans 2027: HMO And PPO Options

Tufts Medicare Preferred sells two network structures, and the choice between them matters more here than with a national carrier because Tufts is a regional plan with a regional network.

Plan type How the network works Best suited to
Tufts Medicare Preferred HMO In-network care only except emergencies. Primary care physician coordinates your care. People whose doctors are already in the Tufts network and who stay in New England
Tufts Medicare Preferred PPO In and out-of-network care, with higher cost sharing outside the network People who travel, winter out of state, or want specialist access without referrals

The HMO side carries several variants built around how much prescription coverage you want and how much premium you are willing to pay. Our broader guides to Medicare Advantage HMO plans and Medicare Advantage PPO plans explain how each structure behaves in practice.

Tufts Medicare Preferred discontinued its $0 Access PPO and replaced it with PPO RX priced at $20 to $60 per month by county

The $0 PPO Tufts Discontinued, And Why It Still Matters

MEMBERS WERE NOT MOVED AUTOMATICALLY

Point32Health discontinued the $0 premium Tufts Medicare Preferred Access (PPO) plan. Members were notified that it would not be offered and told to choose another plan themselves during open enrollment. Nobody was auto-enrolled into the replacement. If you held that plan and did not act, you did not roll into the new PPO by default.

The replacement is Tufts Medicare Preferred PPO RX, and it carries a monthly premium that varies by where you live.

County PPO RX monthly premium
Suffolk County $60
Middlesex County $40
All other counties $20

These are the figures that applied when the plan replaced the discontinued $0 PPO. Tufts Medicare Preferred PPO RX is offered again for 2027 across most of Tufts’s Massachusetts counties, and its 2027 premium and benefits may differ from these. Check your own ZIP code for what is actually available to you.

The reason this is worth your attention going into 2027: a carrier that has already moved a flagship plan from $0 to a county-tiered premium, and that let members lapse rather than migrating them, is a carrier whose annual notice you need to open rather than file. Our guide to Medicare Advantage plans going away in 2027 covers what to do if your plan is one of them.

Tufts Medicare Preferred Plan Lineup

Tufts organizes its Medicare Advantage plans into a handful of families. For 2027, CMS plan data lists these Tufts Medicare Preferred plans, and the naming tells you most of what you need to know.

HMO Basic Rx and Basic No Rx. The entry plans. Basic No Rx has no built-in prescription drug coverage and is intended for people who get drug coverage elsewhere, such as through an employer or the VA.

HMO Smart Saver Rx. A lower-premium HMO with drug coverage included and higher cost sharing when you use care.

HMO Value Rx and Value No Rx. The middle tier, with and without built-in drug coverage.

HMO Prime Rx, Prime Rx Plus and Prime No Rx. The richer HMOs, with lower cost sharing at the point of care in exchange for a higher premium.

PPO RX. The out-of-network option, priced by county as shown above.

Plan availability and the exact names offered depend on your county. Not every family is sold everywhere in the service area, and the specific benefits inside each one may change for 2027.

2027 STAR RATINGS

CMS released the 2027 star ratings on October 8, 2026. Tufts’s main HMO contract (H2256) was for years one of New England’s 5-star plans, but it is not among the 15 contracts rated 5 stars for 2027. That does not make it a weak plan, but it does mean the 5-Star Special Enrollment Period does not apply to it. Check the exact 2027 rating on any Tufts plan you are considering, and see the full 2027 Medicare Advantage star ratings.

Tufts Medicare Preferred Medicare Advantage service area covering Massachusetts and Rhode Island, with Harvard Pilgrim Stride serving New Hampshire and Maine

Where Tufts Medicare Preferred Is Available

For 2027, CMS plan data shows Tufts Medicare Preferred plans in 10 Massachusetts counties: Barnstable, Bristol, Essex, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk and Worcester. No Tufts Medicare Advantage plan is listed in Berkshire, Franklin, Dukes or Nantucket counties, or in Rhode Island. It is not a national carrier and does not sell Medicare Advantage outside New England.

If you live in New Hampshire or Maine, the plan you are probably looking for is Harvard Pilgrim Stride, which is the other Medicare Advantage brand under the same parent company, Point32Health. Same corporate owner, different plans, different networks, different service area. Searching for Tufts in northern New England will not turn up what you need.

CHECK BY ZIP CODE, NOT BY STATE

Medicare Advantage service areas are defined county by county, not state by state. Two neighbouring towns in the same Massachusetts county can have different plans available and different premiums — the PPO pricing above is the clearest example. Enter your ZIP code to see what Tufts actually offers where you live.

Senior Care Options And Special Needs Plans

Beyond the standard Medicare Advantage lineup, Tufts offers coverage for people who qualify for both Medicare and MassHealth.

Tufts Health Plan Senior Care Options is a Massachusetts program that combines Medicare and MassHealth benefits into one plan for people 65 and over who qualify for both. It typically carries no premium for those who qualify and includes care coordination that standard Medicare Advantage plans do not.

Tufts Health One Care has been transitioning to an HMO Dual Eligible Special Needs Plan structure. If you are dually eligible, this is the lane to look at rather than the standard HMO or PPO plans. Our guide to Medicare Advantage Special Needs Plans explains how the three types of special needs plans differ.

How To Choose A Tufts Medicare Preferred Plan For 2027

1

Open your Annual Notice of Change. Tufts has already shown it will discontinue a plan and leave members to re-enroll themselves. That notice is the only document that tells you what is happening to your specific plan.

2

Decide HMO or PPO first. If you spend winters out of state or see specialists outside the Tufts network, the PPO premium buys you something real. If you do not, it does not.

3

Check your doctors by name, not by hospital. Regional plans have tighter networks than national carriers. A hospital being in network does not mean every physician practising there is.

4

Check your drugs against the formulary. The difference between HMO Basic No Rx, Smart Saver Rx and Prime Rx comes down to drug coverage and cost sharing. Picking the wrong one costs more than the premium difference.

5

Compare against the other carriers in your county. Tufts is strong in eastern Massachusetts but it is not the only option. See how it stacks up in our 2027 Medicare Advantage plans by carrier comparison.

6

Act before December 7. The Annual Enrollment Period runs October 15 through December 7. Coverage chosen during it starts January 1.

If you want a wider view before committing, our rankings of the best Medicare Advantage plans for 2027 and the 2027 Medicare Advantage star ratings put Tufts in context against both national and regional competitors.

Tufts Medicare Preferred 2027 FAQ

➤ What is Tufts Medicare Preferred?

Tufts Medicare Preferred is the Medicare Advantage product line sold by Point32Health, the parent company created when Tufts Health Plan and Harvard Pilgrim Health Care merged. For 2027 it offers HMO and PPO Medicare Advantage plans in 10 Massachusetts counties, along with Senior Care Options coverage for people who qualify for both Medicare and MassHealth.

➤ Did Tufts discontinue its $0 PPO plan?

Yes. The $0 premium Tufts Medicare Preferred Access (PPO) plan was discontinued and replaced with Tufts Medicare Preferred PPO RX, which carries a monthly premium that varies by county — $60 in Suffolk County, $40 in Middlesex County and $20 in all other counties. Members of the discontinued plan were not automatically enrolled in the replacement; they were notified and told to choose a plan themselves during open enrollment.

➤ What states does Tufts Medicare Preferred cover?

Massachusetts. For 2027, CMS plan data lists Tufts Medicare Preferred plans in 10 Massachusetts counties and none in Rhode Island. Tufts Medicare Preferred is a regional plan and does not sell Medicare Advantage outside New England. If you live in New Hampshire or Maine, the equivalent plan from the same parent company is Harvard Pilgrim Stride, which has its own separate network and service area.

➤ What is the difference between HMO Prime Rx and Smart Saver Rx?

Both include prescription drug coverage. Smart Saver Rx carries a lower monthly premium with higher cost sharing when you actually use care, while Prime Rx costs more each month and charges you less at the point of care. If you see doctors often or take several prescriptions, the richer plan usually works out cheaper across a full year. HMO Basic No Rx is a third option without built-in drug coverage, intended for people who already have creditable drug coverage elsewhere.

➤ Is Tufts Medicare Preferred the same as Tufts Health Plan?

Tufts Medicare Preferred is the Medicare Advantage brand within Tufts Health Plan, which is itself now part of Point32Health following the merger with Harvard Pilgrim Health Care. If you are shopping Medicare, Tufts Medicare Preferred is the name on the plans you want. Tufts Health Plan also sells commercial and Medicaid coverage that has nothing to do with Medicare.

➤ What are Tufts Health Plan Senior Care Options?

Senior Care Options is a Massachusetts program for people aged 65 and over who qualify for both Medicare and MassHealth. It combines both sets of benefits into a single plan with care coordination built in, and typically carries no premium for those who qualify. It is a different product from the standard Tufts Medicare Preferred HMO and PPO plans, and you have to be dually eligible to enrol.

➤ When can I enroll in a Tufts Medicare Preferred plan for 2027?

The Annual Enrollment Period runs October 15 through December 7, and coverage you choose during it begins January 1, 2027. You can also enroll when you first become eligible for Medicare, or during a Special Enrollment Period if you move, lose other coverage, or your current plan is discontinued. If your Tufts plan is ending, that discontinuation itself opens a Special Enrollment Period.

New to Medicare?

If you are new to Medicare, then you can enroll in a 2027 Medicare Advantage plan up to three months prior to your Medicare Part B effective date.
