Medicare Advantage Plans in New York 2027: Carriers, Costs & Counties

New York is really two Medicare Advantage markets. Downstate and New York City are served by large local plans built around city hospital networks, alongside the national carriers. Upstate is a different world: three regional not-for-profit plans compete county by county, and which of them you can buy from depends entirely on where you live.

2026 was a hard year for all three upstate carriers, and every one of them narrowed what it sells. Below is who sells where, what changed, and what to check before you enroll for 2027 when plan details publish on October 1, 2026.

Who Sells Medicare Advantage In New York

Downstate, plan choice per county is usually the widest in the state. Large local plans compete directly with UnitedHealthcare, Aetna and Humana, and most New York City counties have dozens of plans available. Upstate, the picture narrows to a handful of carriers per county, and the regional plans usually have the stronger local hospital relationships.

The national carriers all sell here, and so do several large local plans whose networks are built around New York City hospital systems. If you see specialists across more than one system, check the network before the premium — city networks vary more than the plan summaries suggest. UPSTATE NEW YORK

Three regional not-for-profits carry most of the market: MVP Health Care across 55 counties, Excellus BlueCross BlueShield across 31, and CDPHP across 18 in and around the Capital Region. Their footprints overlap heavily, so most upstate counties have a real choice between at least two of them.

Plan type is decided for you in some regions. CDPHP sells HMO plans only. Excellus sells HMO in Rochester and PPO everywhere else. MVP sells both in most of its area. If out-of-network coverage matters to you, that narrows the list before you compare a single premium.

HMO vs PPO in New York: Which You Can Actually Get

In most of the country this is a preference. In New York it is often decided by your county before you get a say. CDPHP sells HMO plans only. Excellus sells HMO and HMO-POS in the Rochester region and PPOs everywhere else. MVP sells both across most of its footprint. Downstate, both types are widely available.

HMO AND HMO-POS

You use the plan’s network and name a primary care doctor who coordinates referrals. Out-of-network care is generally not covered except in an emergency. HMO-POS plans allow some out-of-network care at a higher cost share, usually with its own yearly limit. In exchange, these plans carried the lowest out-of-pocket maximums in New York in 2026 — as low as $6,000 on one MVP plan. See our guide to Medicare Advantage HMO plans 2027.

PPO

You can see out-of-network providers without a referral, at a higher cost share. Every PPO carries two spending caps: one for in-network care and a higher combined cap for in-network and out-of-network together. The combined figure ran as high as $13,900 on New York PPOs in 2026, and it is the number most people never check. See our guide to Medicare Advantage PPO plans.

Snowbirds and second homes. If you spend part of the year outside New York, an HMO is the harder fit. Emergency care travels with you on any plan, but routine care generally does not. A PPO, or a plan with a national network, is usually the better trade.

What Changed For New Yorkers, And Why

Upstate New York took an unusual hit. A 2023 change to the Medicare wage index lowered hospital payment rates in the region, and the regional not-for-profit plans absorbed most of it. Excellus says that change alone costs it about $100 million a year. CDPHP cited the same issue when it cut its service area from 33 counties to 18 and dropped its PPO plans entirely. MVP pointed to rising costs plus the new cap on member drug spending, which shifts drug costs from members onto plans.

If you are upstate, check that your plan still exists. This is not the usual advice to compare plans. Three of the largest upstate carriers narrowed their lineups or service areas in a single year, and your Annual Notice of Change, mailed by September 30, is what tells you whether yours survived.

There is also a merger in progress. CDPHP and Excellus now share a parent company, and a full merger of CDPHP into Excellus Health Plan is planned for early 2027 subject to New York State approval. Both have said CDPHP keeps its local brand for most members. For your coverage, what matters is the specific plan sold in your county, not the corporate structure above it.

Five checks before enrolling in a New York Medicare Advantage plan for 2027

What Medicare Advantage Costs in New York for 2027

Plan premiums for 2027 are not public until October 1, 2026. What is already set are the federal numbers that apply to every New Yorker on Medicare, whichever plan you pick.

You pay the Part B premium no matter which Medicare Advantage plan you join. That is the floor. On top of it sits the plan premium, which in New York ran from $0 to about $229 a month in 2026 depending on the carrier and county.

The premium and the out-of-pocket maximum move in opposite directions. In New York in 2026, the $0 premium plans generally carried out-of-pocket maximums around $7,200 to $9,250, while plans charging $116 to $229 a month brought that down to $6,000 to $6,800. If you expect a heavy year of care, the higher premium is often the cheaper plan.

Add the three together — plan premium for the year, the out-of-pocket maximum you could realistically hit, and your drug costs — and compare that total. Comparing premiums alone is how people end up in the wrong plan.

Is your plan still sold in your county for 2027? Your Annual Notice of Change answers this, and upstate it is the first thing to check.

Are your doctors and hospital in network? Call the office and ask about the specific plan for 2027, not the carrier in general.

What is the in-network out-of-pocket maximum? That number, not the premium, is what a bad year costs you. Check the plan’s 2027 Medicare Advantage star ratings too.

Did the extra benefits shrink? Compare the 2027 dental, over-the-counter and eyewear allowances against what you actually used this year.

Enroll October 15 to December 7, 2026. Coverage starts January 1, 2027. If you are already in a Medicare Advantage plan, you get one more switch between January 1 and March 31, 2027. If your plan is discontinued, you get a special enrollment period of your own.

Plan availability is set at county level, so where you live inside New York changes the answer more than anything else. Here is the shape of each market.
