Providence Medicare Advantage Plans 2027: Coverage, Costs & Star Ratings

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated September 28, 2026

Providence Health Assurance is the Medicare arm of Providence Health Plan, a division of the nonprofit Providence health system. It is deliberately small. Rather than chase counties, Providence sells only where its own hospitals and physician groups are strongest, which is why it covers thirteen counties across three states and nothing beyond them.

That focus shows up in the quality scores: 4 out of 5 stars from CMS, held even in a year when performance thresholds rose across most measures. It also means the plan sold in your county may look nothing like the one sold two hundred miles away. Below is every plan, every number, and how to tell which is right for you when 2027 details publish on October 1, 2026.

Providence Medicare Advantage Plans 2027: At A Glance

Measure Providence
Star rating 4 out of 5 from CMS on contract H9047
States Oregon, Washington and Orange County, California
Counties Thirteen, not statewide in any of the three
Plan types HMO, HMO-POS and HMO special needs plans — no PPO
Owned by Providence Health Plan, part of a nonprofit health system
Referrals Not required for in-network specialists, except on California plans

These figures reflect the current plan year. 2027 Providence plans, premiums, out-of-pocket maximums and benefits become viewable on October 1, 2026 and may change. Not all plans offer all benefits, and availability varies by county.

The counties served are Clackamas, Hood River, Lane, Multnomah, Washington and Yamhill in Oregon; Clark, Snohomish, Spokane, Walla Walla, Benton and Franklin in Washington; and Orange County in California. If you are outside those, Providence is not an option however good the rating looks.

EVERY PLAN IS AN HMO

Providence offers no PPO. Its contracts are HMO, HMO-POS and HMO special needs plans, so out-of-network care is generally not covered outside emergencies. If you travel often or keep a second home, look at a carrier with a PPO in your county instead.

Providence Medicare Advantage annual cost compared across the three plans in a quiet, average and bad year

Three Plans, Three Different Markets

Providence does not run one lineup across its footprint. Each named plan serves a different set of counties, and the numbers behind them are nothing alike.

Plan Premium Out-of-pocket max Part B giveback Where it is sold
Sycamore + Rx (HMO) $0 $400 $10 a month Orange County, CA
Pine + Rx (HMO) $0 $6,750 $24 a month Benton, Franklin, Snohomish, Spokane, Walla Walla, WA
Extra + Rx (HMO) $161 $4,200 None Six Oregon counties plus Clark, WA

THAT $400 CAP IS NOT A TYPO

Sycamore + Rx carries a $0 premium, $0 primary care copays, $0 specialist copays, a $10 monthly Part B giveback and a $400 in-network out-of-pocket maximum. That is one of the lowest caps in the country, and it is sold in exactly one county. If you live in Orange County, California, it deserves a hard look.

At the other end, Pine + Rx also costs $0 a month and pays back $24 of your Part B premium, but its cap is $6,750 and its Part D deductible is $195, waived on tiers 1 and 2. Extra + Rx charges $161 a month and buys the middle ground: a $4,200 cap, no drug deductible and the richest extras of the three.

Providence also runs a dual-eligible special needs plan for members in Portland and surrounding areas, with enhanced care coordination, hearing aid coverage, reduced chiropractic costs, rollover over-the-counter benefits, waived blood service deductibles and up to 24 one-way trips a year in transportation. That transportation benefit is limited to members with qualifying chronic conditions, and eligibility requirements apply. Our guide to Medicare Advantage special needs plans explains how these work.

Extra Benefits Differ Sharply Between Plans

Benefit Sycamore + Rx Pine + Rx Extra + Rx
Dental $350 flex card every 6 months $0 preventive, optional buy-up $0 preventive, optional buy-up
Eyewear allowance Not listed $100 a year $150 a year
Over-the-counter Not covered Not covered $240 every 6 months
Specialist copay $0 $45 $20
Part D deductible None $195, waived tiers 1 and 2 None
Hearing aids $499 standard, $699 advanced, $999 premium, per aid, on all three plans
Fitness $0 gym membership on all three plans, more than 17,000 clubs

The dental flex card exists on one plan only. Sycamore’s $350 every six months can go toward fillings, crowns or root canals, the things preventive coverage never pays for. Pine and Extra cover cleanings, exams and X-rays at $0 and charge an extra premium for anything beyond that. If dental is what you are shopping for, our guide to Medicare Advantage plans with dental coverage explains how those allowances actually pay out.

The 4-Star Rating, And What Earned It

CMS rated Providence 4 out of 5 stars on contract H9047, above the average for the industry and better than most national carriers. It held that rating in a year when CMS raised performance thresholds across most of the forty-plus measures the score is built from.

The standouts were a perfect 5 stars for Part D call centre language and accessibility services, plus high marks for controlling blood pressure and monitoring physical activity. Those last two are exactly the measures a health system that owns its own clinics can move, which is the recurring pattern behind provider-owned plans outscoring national ones. Our guide to the 2027 Medicare Advantage star ratings explains what the score does and does not measure.

A rating is not a network check. Providence scoring 4 stars tells you nothing about whether your cardiologist is in it. Every plan here is an HMO, so confirm each doctor you actually see before you enrol, not just your primary.

How Providence Compares

Against the national carriers, this is the standard regional trade: Providence gives you a higher star rating and a tighter local network, while a national plan gives you coverage that travels and a PPO option Providence does not sell. If you want the field ranked, our comparison of the best Medicare Advantage plans 2027 scores every major carrier on ratings, stability and availability.

If you need a network that follows you out of state, look at UnitedHealthcare Medicare Advantage plans 2027 or Aetna Medicare Advantage plans 2027, both of which sell PPOs across most of the country. If price is the deciding factor, Wellcare Medicare Advantage plans 2027 run about 47% $0-premium, though at lower star ratings.

How To Choose A Providence Plan For 2027

YOUR 2027 CHECKLIST

Five steps before you enrol

1

Check your county first. Thirteen counties across three states, and each plan is sold in only some of them. Outside that list, Providence is not available at all.

2

Confirm every doctor, not just your primary. All plans are HMOs, so a specialist outside the network is generally not covered except in an emergency.

3

Compare the cap, not the premium. The $0 plans range from a $400 cap to a $6,750 one. That gap matters far more than the monthly figure.

4

On October 1, 2026, price your prescriptions. Use the Medicare Plan Finder tool and enter every drug with its dose and your actual pharmacy. Pine carries a $195 drug deductible the other two do not.

5

Enrol between October 15 and December 7, 2026. Coverage starts January 1, 2027.

Already a Providence member? Providence has said it is concentrating resources on the markets where its provider partnerships are strongest, which means some service areas change from year to year. Read your Annual Notice of Change when it arrives — it tells you whether your plan continues in your county and what happened to your costs.

Providence Medicare Advantage FAQs

➤ Is Providence a good Medicare Advantage plan?

On quality, yes. CMS rated Providence 4 out of 5 stars on contract H9047, above the industry average, and it held that score in a year when thresholds rose across most measures. The limits are reach and plan type: thirteen counties across three states, and every plan is an HMO, so there is no out-of-network coverage outside emergencies.

➤ What states and counties does Providence cover?

Thirteen counties across three states: Clackamas, Hood River, Lane, Multnomah, Washington and Yamhill in Oregon; Clark, Snohomish, Spokane, Walla Walla, Benton and Franklin in Washington; and Orange County in California. It is not statewide in any of them, and each plan is sold in only part of that footprint.

➤ Does Providence offer a PPO plan?

No. Providence Health Assurance holds HMO, HMO-POS and HMO special needs plan contracts only. Out-of-network care is generally not covered outside emergencies, so if you travel frequently or keep a second home you will want a carrier that sells a PPO in your county.

➤ Which Providence plan has the lowest out-of-pocket maximum?

Sycamore + Rx, at $400 in network — one of the lowest caps offered by any carrier anywhere. It also carries a $0 premium, $0 primary care and specialist copays and a $10 monthly Part B giveback, but it is sold only in Orange County, California. Pine + Rx sits at $6,750 and Extra + Rx at $4,200.

➤ Does Providence have a Part B giveback?

Two of the three plans do. Pine + Rx returns $24 a month and Sycamore + Rx returns $10 a month, paid as a reduction in what you owe Social Security for Part B. Extra + Rx has no giveback. Giveback amounts are reset every year, so confirm the 2027 figure on October 1, 2026 rather than assuming it carries over.

➤ Does Providence include dental coverage?

All three plans cover preventive dental — exams, cleanings, X-rays and fluoride — at $0, with limits. Only Sycamore + Rx includes a flex dental card, worth $350 every six months toward services of your choosing such as fillings, crowns or root canals. Pine and Extra offer dental beyond preventive as an optional buy-up for an additional premium.
