Medicare Advantage Plans in Hawaii 2027: HMSA Changes, Carriers and Premiums

HMSA is closing two plans. Akamai Advantage Complete Plus and Standard Plus close December 31, and about 16,000 members were notified, according to the Honolulu Star-Advertiser. New versions with the same names cost $240 a month.

HMSA premiums are going up. The Complete PPO on Oahu rises to $55 a month from $20, and the Standard PPO on the neighbor islands rises to $50 from $20.

New HMSA HMO, without Queen’s. HMSA’s new Select Plus HMO on Oahu costs $45 a month, but Queen’s says its hospitals and clinics will be out of network for that plan.

More plans overall. Hawaii has 46 Medicare Advantage plans for 2027, up from 44, and CMS reports 100% of Hawaii residents with Medicare have access to a $0 premium plan.

9 Part B giveback plans that may lower your Part B premium, from Devoted and Humana.

Dual plans (D-SNPs) for people with Medicare and Med-QUEST come from Kaiser, UnitedHealthcare, AlohaCare and HMSA Dual Care.

Chronic condition plans (C-SNPs) come from Devoted.

Prescription drug coverage, with Part D out-of-pocket costs capped at $2,400 in 2027.

Dental, vision and hearing benefits in many plans.

Part B premium reductions in giveback plans, where offered.

Fitness and over-the-counter allowances in some plans.

If you got a letter that your HMSA plan won’t be offered in 2027, act now. Pick new coverage during Annual Enrollment so you’re covered January 1.

Check your doctors and hospitals in each plan’s network, including The Queen’s Health System, Hawai’i Pacific Health, Kaiser Permanente and Adventist Health Castle.
