Keystone 65 is the Medicare Advantage brand of Independence Blue Cross, the dominant health insurer in southeastern Pennsylvania. If you live in or around Philadelphia and you are shopping Medicare, it is probably the first name you were given.

There is one thing to understand before you compare anything: every Keystone 65 plan is an HMO. If you want a PPO from Independence Blue Cross, it exists, but it is not called Keystone 65 — and that single fact sends people down the wrong path every year.

Keystone 65 HMO Plans 2027: Select, Basic, Preferred And Focus

Independence Blue Cross sells Keystone 65 as a family of HMO plans that differ mainly in network breadth and whether prescription drug coverage is built in.

Independence also offers an HMO-POS variant, which behaves like an HMO but allows limited out-of-network care at higher cost. Plan names, premiums and benefits are refiled every year, so confirm what is actually sold at your address for 2027 rather than assuming the lineup carries over.

Focus is the one to read carefully. A tiered network plan prices your care differently depending on which tier your doctor or hospital sits in. The premium is lower, but a specialist in the wrong tier can cost considerably more than the same specialist would on Basic or Select. Check your own physicians by tier, not just by whether they are in network at all.

Personal Choice 65: The PPO That Keystone 65 Is Not

THERE IS NO KEYSTONE 65 PPO

Every Keystone 65 plan is an HMO. Independence Blue Cross does sell a Medicare Advantage PPO, but it is branded Personal Choice 65 — a different name, a different product page, a different enrollment path. If you have been searching for a Keystone 65 PPO and finding nothing, this is why.

The practical difference matters more than the branding. On Keystone 65 you choose a primary care physician who coordinates your care and you generally need a referral to see a specialist. On Personal Choice 65 you do not need referrals, and you can go out of network at higher cost sharing.

If you winter somewhere warm, split time between homes, or see specialists at hospitals outside the Independence network, the PPO is the product you want and the HMO will frustrate you. Where Keystone 65 Is Available

Keystone 65 Medicare Advantage service area covering Philadelphia, Bucks, Chester, Delaware and Montgomery counties in southeastern Pennsylvania

Keystone 65 is sold in five southeastern Pennsylvania counties: Philadelphia, Bucks, Chester, Delaware and Montgomery. That is the whole service area.

If you are anywhere else in Pennsylvania, Keystone 65 is not an option for you. Pennsylvania is one of the more sharply divided Medicare Advantage markets in the country. The carrier that dominates Philadelphia has no presence in Pittsburgh and vice versa. If you are moving within the state, your plan almost certainly does not move with you — and a move out of your plan’s service area opens a Special Enrollment Period so you can switch.

What Keystone 65 Plans Include

Independence has built Keystone 65 around a few consistent features, though not every plan carries every one.

  • $0 copay for in-network primary care visits. Available across the Keystone 65 HMO plans, which removes the cost barrier to routine care.
  • Virtual care through Teladoc Health. Telehealth access without needing an in-person appointment first.
  • Round-the-clock nurse health coaches. A 24/7 line for clinical questions between appointments.
  • A quarterly over-the-counter allowance on select plans, for health essentials from an approved catalogue.
  • SilverSneakers fitness membership on select plans.
  • The Choice Program for dental, vision and hearing coverage on select plans.

The phrase “on select plans” is doing real work in that list. Supplemental benefits like over-the-counter allowances and fitness memberships are being trimmed across the Medicare Advantage industry for the 2027 plan year, so confirm which ones attach to the specific plan you are considering rather than assuming the brand carries them all. How To Choose A Keystone 65 Plan For 2027

  1. Decide HMO or PPO before you look at plan names. If you need out-of-network access or referral-free specialist visits, you want Personal Choice 65, not Keystone 65. Getting this wrong wastes the whole comparison.
  2. Confirm you are in one of the five counties. Philadelphia, Bucks, Chester, Delaware or Montgomery. Nothing else qualifies.
  3. Check whether you need built-in drug coverage. Preferred Medical Only exists for people covered elsewhere. If that is not you, it is the wrong plan however attractive the premium looks.
  4. If you are considering Focus, check your doctors by tier. Not just whether they are in network — which tier they sit in, because that is what sets your cost.
  5. Check your prescriptions against the formulary. Drug tiers shift every year, and a tier change usually costs more than the premium difference between two plans.

If you see doctors regularly or take several prescriptions, the higher-premium plan often works out cheaper across a full year.

Focus is a tiered network version of Keystone 65. Instead of one in-network price, your cost depends on which tier your doctor or hospital falls into, and the premium is lower in exchange for that narrower preferred tier. It suits people whose existing physicians already sit in the preferred tier. Check your own doctors by tier before enrolling, because a specialist in the wrong tier can cost significantly more than on Basic or Select.

Dental, vision and hearing coverage comes through the Choice Program, which is available on select Keystone 65 plans rather than all of them. SilverSneakers fitness membership and the quarterly over-the-counter allowance are also limited to select plans.
