Keystone 65 is the Medicare Advantage HMO brand sold by Independence Blue Cross in southeastern Pennsylvania. The family includes Keystone 65 Basic Rx, Select Rx, Preferred Medical Only and Focus Rx, along with an HMO-POS variant. All of them are HMO plans, so you choose a primary care physician and generally need a referral to see a specialist.
➤ Is there a Keystone 65 PPO?
No. Every Keystone 65 plan is an HMO. Independence Blue Cross does sell a Medicare Advantage PPO, but it is branded Personal Choice 65 rather than Keystone 65. If you want out-of-network coverage or specialist access without referrals, Personal Choice 65 is the product to look at — it is the same insurer under a different plan name.
➤ What counties does Keystone 65 cover?
Five counties in southeastern Pennsylvania: Philadelphia, Bucks, Chester, Delaware and Montgomery. Keystone 65 is not sold anywhere else in the state. If you live in central or western Pennsylvania you will be looking at different carriers entirely, and if you move out of those five counties your plan does not move with you.
➤ What is the difference between Keystone 65 Basic and Select?
Both are HMO plans with prescription drug coverage built in. Basic Rx carries the lowest premium in the Keystone 65 family, with higher cost sharing when you use care. Select Rx sits above it, trading a higher monthly premium for lower costs at the point of care. If you see doctors regularly or take several prescriptions, the higher-premium plan often works out cheaper across a full year.
➤ What is Keystone 65 Focus?
Focus is a tiered network version of Keystone 65. Instead of one in-network price, your cost depends on which tier your doctor or hospital falls into, and the premium is lower in exchange for that narrower preferred tier. It suits people whose existing physicians already sit in the preferred tier. Check your own doctors by tier before enrolling, because a specialist in the wrong tier can cost significantly more than on Basic or Select.
➤ Does Keystone 65 include dental and vision?
Dental, vision and hearing coverage comes through the Choice Program, which is available on select Keystone 65 plans rather than all of them. SilverSneakers fitness membership and the quarterly over-the-counter allowance are also limited to select plans. Supplemental benefits like these are being reduced across the industry for 2027, so confirm which ones attach to the specific plan you are considering.

Generally yes. Keystone 65 plans are HMOs, which means you select a primary care physician who coordinates your care and refers you to specialists. The HMO-POS variant allows some out-of-network care at higher cost. If referral-free specialist access matters to you, Personal Choice 65, the Independence Blue Cross PPO, does not require them.
➤ When can I enroll in Keystone 65 for 2027?
The Annual Enrollment Period runs October 15 through December 7, and coverage you choose during it begins January 1, 2027. You can also enroll when you first become eligible for Medicare, or during a Special Enrollment Period — including if you move into or out of the five-county service area, lose other coverage, or your current plan is discontinued.
