MVP Health Care is a not-for-profit insurer based in Schenectady, and it sells Medicare Advantage across 55 New York counties in four regions: the Capital District and Central New York, the Hudson Valley, Rochester and Buffalo, and the North Country. It is one of the few carriers competing in all of those markets at once.

MVP also made a decision for 2026 that few carriers announce openly: it stopped offering $0 premium plans in the Rochester market. MVP sells different plans, at different prices, in each of its four regions. The table below is the 2026 lineup, which is the best available guide to 2027 until new plan details are released.

These are 2026 figures. 2027 MVP plans, premiums and benefits become viewable on October 1, 2026 and may change. Plan names, out-of-pocket limits and drug deductibles can all shift between years. Not all plans offer all benefits.

Two things stand out. The same plan costs wildly different amounts depending on region: Medicare Complete Wellness was $15 a month in the Capital District and Hudson Valley, and $174 in Northern New York. And the only $0 premium plan, Medicare Preferred Gold without Part D, carries no drug coverage at all, so you would need a separate Part D plan alongside it. MVP covers 55 New York counties. It is one of the widest regional footprints in the state, reaching from Buffalo to Westchester.

That footprint overlaps heavily with the other upstate carriers, which is good news if you are shopping. In the Capital Region you can weigh MVP against CDPHP Medicare Advantage plans 2027, and in Rochester, Central New York and the North Country against Excellus Medicare Advantage plans 2027. Comparing all three in your county is worth the half hour it takes.

Northern New York has the thinnest choice. Two MVP plans there in 2026 against three elsewhere, and the PPO cost $174 a month instead of the $15 charged downstate for the same plan name.

Why MVP Health Care dropped $0 premium Medicare Advantage plans in Rochester

What Changed For 2026, And What It Could Mean For 2027

In October 2025, MVP announced that it would no longer offer $0 premium Medicare Advantage plans in the Rochester market beginning in 2026. It gave three reasons: rising health care costs, federal policy changes, and plan consolidation.

The federal piece is the one most people have not heard about. The cap on what members pay out of pocket for drugs under Part D, $2,100 in 2026, is genuinely good for patients, but it moves a large share of drug costs from the member onto the plan. Every carrier is absorbing that, and it is a major reason premiums rose and benefits thinned across the industry.

Read your Annual Notice of Change. MVP mails it by September 30. It is the only document that tells you exactly what changes in your specific plan for 2027, including premium, copays, drug coverage and whether the plan continues at all.

MVP’s own framing was blunt. Its chief market strategy officer said the company is “not here to chase irrational pricing” but to build coverage that lasts. Read that as a signal: MVP is more likely to price a plan honestly than to advertise a $0 premium it cannot sustain. It also means you should not assume the 2026 lineup carries over unchanged into 2027.

MVP is not alone. Excellus ended select plans and CDPHP cut its service area from 33 counties to 18, both for the same 2026 plan year. Upstate New York lost a lot of Medicare Advantage choice in one season.

MVP HMO-POS Vs PPO: How They Differ

MVP sells both plan types in most of its regions, so unlike some regional carriers, you generally get a choice.

PREFERRED GOLD AND SECURE PLUS · HMO-POS

You use the MVP network and coordinate care through a primary care doctor, and the point-of-service feature allows some out-of-network care at a higher cost share. COMPLETE WELLNESS AND WELLSELECT · PPO

You can see out-of-network providers without a referral, at a higher cost share. MVP’s PPOs carry two spending caps: $9,250 in network and $13,900 for in-network and out-of-network care combined in 2026. MVP’s dental allowance is unusually flexible, and it is the benefit most worth checking when 2027 details publish. 2027 plans may include similar extras, depending on the plan and area, but nothing is confirmed yet.
