HumanaChoice PPO Plans 2027: Giveback, Special Needs Versions and Where It's Sold

HumanaChoice is also sold in parts of Alabama, Pennsylvania, Wisconsin, Louisiana, West Virginia, New York, Colorado, South Carolina, Florida, Kansas, Montana, Washington, New Mexico, Nebraska, Idaho, Utah, New Jersey, California, Maine, Maryland, Arizona, Oregon, Nevada, Wyoming, Massachusetts, New Hampshire, Vermont, Connecticut, Delaware, Hawaii and Washington, DC. In Michigan, only the special needs versions are listed for 2027. Not every plan is sold in every county, so check by ZIP code.

HumanaChoice SNP-DE plans are for people with both Medicare and Medicaid, and HumanaChoice Diabetes and Heart plans are for people with qualifying chronic conditions. Star ratings are given to each Medicare contract, not to a plan name. 133 of the 204 HumanaChoice plans are on Humana’s national PPO contract, H5216, which rose from 3.5 to 4 stars for 2027. The rest are on five other Humana contracts.

Across all its Medicare Advantage plans, Humana reports that 95% of its members are in plans rated 4 stars or higher for 2027. The national average rating for 2027 is 3.99. HumanaChoice is Humana’s PPO line. Humana Gold Plus is Humana’s HMO line. A PPO lets you see out-of-network providers and usually doesn’t require referrals, while an HMO generally keeps you in network and may cost less.
