Regence is the Blue Cross Blue Shield licensee for the Pacific Northwest and parts of the Mountain West, selling Medicare Advantage in Idaho, Oregon, Utah and select Washington counties. It covers roughly 73,000 Medicare Advantage members, which makes it a genuinely small carrier compared with the national names.

Regence is worth a close look if you want the Blue network in a region where options are thin. It is also a carrier where the premiums and the star ratings deserve more scrutiny than most. This page covers both.

Where Regence Medicare Advantage Is Available

Regence operates as four separate Blue licensees, and the plans differ between them.

  • Oregon — Regence BlueCross BlueShield of Oregon, across a band of western and southern counties including the Portland metro, Willamette Valley and Rogue Valley
  • Washington — Regence BlueShield, in select counties only. This is not statewide coverage, and the county list has contracted in recent years
  • Idaho — Regence BlueShield of Idaho, concentrated in the Boise area and the northern panhandle
  • Utah — Regence BlueCross BlueShield of Utah, across the Wasatch Front and several southern counties

Regence has pulled out of counties before. It discontinued PPO plans across more than a dozen Washington counties and several Oregon counties in recent plan years, and withdrew some HMO plans entirely. If you live in a rural county in its service area, confirm Regence still sells where you live for 2027 rather than assuming it does.

Regence Medicare Advantage Plans for 2027

Regence sells mostly PPOs, with a smaller number of HMOs in specific markets.

MEDADVANTAGE + RX — PRIMARY, CLASSIC, ENHANCED (PPO)

The core PPO line, sold in all four states and tiered by how much you pay each month in exchange for lower copays. Primary sits at the bottom, Enhanced at the top. Premiums differ between metro and non-metro versions of the same plan.

VALIANCE (PPO)

Regence’s Oregon $0 premium option, offered across a wide group of western and southern Oregon counties. If a $0 premium is what you are after from Regence, this has historically been the plan to look at first.

HMO PLANS

A smaller HMO lineup in specific markets, including plans built around local health systems in Idaho. Tighter networks and usually lower cost, but availability is narrow and Regence has discontinued HMO plans in some counties in recent years.

Specific 2027 premiums, copays and supplemental benefits vary by plan and county, and not every plan offers every benefit. Plan names and availability can change year to year, so check the documents for your own ZIP code before enrolling.

The Honest Assessment

Most pages about a carrier read like a brochure. Here is what the published data actually shows about Regence, good and bad.

Premiums run high. Regence sells relatively few $0 premium Medicare Advantage plans — in the 2026 plan year only one carried a $0 premium. Across its PPO line, monthly premiums have ranged from the low teens up past $190 depending on the plan, the tier and whether you are in a metro or non-metro county. That is well above what most carriers charge.

No Part B giveback. Some carriers offer a plan that rebates part of your Medicare Part B premium. Regence does not.

Star ratings are below average. Independent reviews of the 2026 ratings put Regence under the national average, with individual plans landing anywhere from 3 to 4 stars depending on the market. Ratings are assigned per contract, so the plan in your county may differ from the company picture.

The Blue network is the real draw. Regence members get nationwide access through the BlueCard network, which matters more than usual in a region where several counties have very few plans at all. A PPO with national Blue access is a genuinely different proposition from a local HMO.

Where Regence makes sense: you want PPO flexibility and national Blue network access, you travel or split time between states, and you are willing to pay a monthly premium to get it. If your priority is the lowest possible premium, compare carefully before assuming the Blue name is worth the difference.

What Is Changing for 2027

Plan details for 2027 publish on October 1, 2026. Until then, nothing Regence-specific is confirmed beyond the federal rules that apply to every carrier.

Regence has a track record of adjusting its county footprint between plan years, so the service area is the first thing to check rather than the last. For the broader picture on carrier exits this year, see our guide to which Medicare Advantage plans are going away in 2027.

Read your Annual Notice of Change. Plans mail it by September 30. It tells you whether your plan continues, and what changes in premium, copays, drug coverage and network. If your plan is ending, you may qualify for a Special Enrollment Period that runs past December 7.

Most plans set limits below the federal ceiling. You continue paying your Medicare Part B premium on top of any Regence plan premium, and because Regence does not offer a Part B giveback, that cost is unavoidable here.

How Regence Compares in the Northwest

Regence is not the only carrier in its four states, and in the larger metros it is far from the obvious choice.

  • If you want the lowest premium — compare against UnitedHealthcare Medicare Advantage plans and Humana Medicare Advantage plans, which offer far more $0 premium options in these markets
  • If star ratings are your priority — Kaiser Permanente Medicare Advantage plans operate in Oregon and Washington and hold the highest ratings in the country, though they run on a closed network model
  • If you want PPO flexibility and national access — this is where Regence competes best, and the BlueCard network is the specific reason

Five steps before you enroll

1. Confirm Regence still sells in your county for 2027. Its footprint has moved in recent years, and rural counties are where it has pulled back.

2. Check the star rating on the specific plan. Regence’s company average sits below the national figure, but individual plans in some counties rate considerably better.

3. Price the premium against what you actually get. Regence charges more than most carriers in these markets — make sure the network or benefits justify it for your situation.

4. Run your prescriptions through the 2027 formulary. Check the tier each drug sits on, not just whether it is covered.

5. Enroll between October 15 and December 7, 2026 for coverage starting January 1, 2027 — unless a Special Enrollment Period applies to you.

➤  What states does Regence Medicare Advantage cover?

Idaho, Oregon, Utah and select counties in Washington. Regence operates as four separate Blue Cross Blue Shield licensees across those states, and the plans differ between them. It does not sell Medicare Advantage anywhere outside these four states.

➤  Does Regence offer $0 premium Medicare Advantage plans?

Very few. In the 2026 plan year only one Regence Medicare Advantage plan carried a $0 premium, the Valiance PPO in Oregon. Most of its plans charge a monthly premium, and Regence does not offer a Part B giveback. If a $0 premium is your priority, compare other carriers in your county first.

➤  Can I use a Regence plan outside the Northwest?

Regence PPO members get nationwide access through the BlueCard network, which is the main argument for choosing Regence over a local HMO. You must still live in the plan’s service area to enrol and stay enrolled.

➤  Do I still pay my Part B premium with Regence?

Yes, and unlike some carriers Regence does not offer a plan that rebates part of it. You must keep Medicare Part A and Part B and keep paying your Part B premium on top of whatever the Regence plan itself costs.
