CarePlus Health Plans 2027: Medicare Advantage Coverage and Costs

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 8, 2026

CarePlus Health Plans is a Florida-only Medicare Advantage carrier and a subsidiary of Humana. It has served Florida Medicare beneficiaries for more than 25 years and covers roughly 158,000 members across about 20 counties.

Two things make CarePlus unusual: every plan it sells carries a $0 monthly premium, and every plan is built on an HMO structure. This page covers the plan families, where they are available, and what the Humana connection means for 2027.

Where CarePlus Health Plans Are Available

Florida and nowhere else. CarePlus does not sell Medicare Advantage in any other state, and it never has.

Within Florida it serves about 20 counties, grouped into five regions:

  • South Florida — Broward, Miami-Dade and Palm Beach
  • Central Florida — Lake, Marion, Orange, Osceola, Seminole, Sumter and Polk
  • West Florida — Hillsborough, Pasco, Pinellas and Hernando
  • North Florida — Clay, Duval and St. Johns
  • Atlantic Coast — Brevard, Flagler, Indian River and Volusia

Plan availability is set county by county. CarePlus has offered up to 12 plan options in a single county, but the specific plans differ between them. A plan name you read about in Miami may not be sold in Jacksonville. Always check your own ZIP code.

CarePlus Plan Families for 2027

CarePlus names its plans rather than numbering them, and the names tell you roughly who each one is for.

CAREONE PLUS (HMO AND HMO-POS)

One of the core plans, sold across several regions. The HMO-POS versions add a point-of-service option, which gives you limited out-of-network coverage that a straight HMO does not.

CAREFREE AND CAREFREE PLATINUM GIVEBACK (HMO)

The giveback plans reduce what you pay toward your Medicare Part B premium each month. Availability is narrow and the giveback amount varies by plan and county, so confirm both before enrolling.

CAREACCESS AND CARESALUTE (HMO)

Broader-footprint HMO plans that have reached across most of CarePlus territory. These tend to be the plans available in the widest range of counties.

CARENEEDS PLATINUM AND CARENEEDS PLUS (HMO D-SNP)

Dual-eligible special needs plans for people who have both Medicare and Florida Medicaid. You must qualify for Medicaid to enrol. CarePlus works with the Florida Department of Children and Families to help members apply for public assistance programs.

CAREBREEZE PLATINUM AND CARECOMPLETE PLATINUM (HMO C-SNP)

Chronic condition special needs plans. CareBreeze has been built around chronic lung conditions including COPD; CareComplete targets other qualifying chronic conditions. You need a qualifying diagnosis to enrol.

Plan names, availability and benefits can change each year, and not every plan offers every benefit. Check the 2027 plan documents for your own county before enrolling.

CarePlus and Humana: What the Connection Means

CarePlus is a wholly owned subsidiary of Humana, but it operates under its own name, its own contract with Medicare, and its own provider network. If you are enrolled in CarePlus, you are not enrolled in a Humana plan.

That distinction matters more than usual this year. Humana is reducing its Medicare Advantage footprint nationally for 2027, with exits expected to affect around 600,000 members across the country. Those are Humana-branded plans under Humana contracts.

Do not assume either way. A Humana exit elsewhere in the country does not automatically affect your CarePlus plan, and a CarePlus plan ending in your county is not necessarily connected to Humana’s national changes. Your Annual Notice of Change is the only document that tells you what is happening to your specific plan.

For the national picture on carrier exits, see our guide to which Medicare Advantage plans are going away in 2027. For the parent company’s own lineup, see Humana Medicare Advantage plans.

The Honest Assessment

What the published data actually shows about CarePlus, good and bad.

Every plan is $0 premium. Not most — all of them, including the special needs plans. Very few carriers can say that across an entire lineup.

Deep plan choice in a small area. Up to 12 plan options in a single county is more granular choice than most national carriers offer anywhere.

Everything is an HMO. There is no true PPO in the CarePlus lineup. The HMO-POS plans add limited out-of-network coverage, but if you want full PPO freedom, CarePlus does not sell it.

Florida only, and only part of Florida. If you move out of state, or even to a Florida county CarePlus does not serve, you lose the plan. For snowbirds who spend months elsewhere, an HMO tied to a Florida network is a real limitation.

Where CarePlus makes sense: you live year-round in one of its Florida counties, your doctors are in its network, and you want a $0 premium plan with strong supplemental benefits. If you travel often or split the year between states, compare a PPO before committing.

Federal Limits That Apply to Every CarePlus Plan

Some 2027 figures are already fixed, because Medicare sets them for every carrier.

What 2027 limit
Medicare Advantage out-of-pocket maximum, in network No plan may exceed $9,850
Part D out-of-pocket cap $2,400
Maximum Part D deductible $700

CarePlus plans have historically set out-of-pocket maximums well below the federal ceiling. A $0 premium plan still means you pay your Medicare Part B premium — the giveback plans reduce that amount but do not eliminate it.

How CarePlus Compares in Florida

Florida is one of the most competitive Medicare Advantage markets in the country, with well over 150 plans from around 19 carriers.

YOUR CAREPLUS CHECKLIST

Five steps before you enroll

1. Confirm CarePlus serves your county. It covers about 20 of Florida’s 67 counties, and plan availability differs within that footprint.

2. Check your doctors against the 2027 directory. These are HMO plans, so out-of-network care generally is not covered except in emergencies.

3. Run your prescriptions through the 2027 formulary. Check the tier each drug sits on, not just whether it is covered.

4. Compare the plans available in your own county side by side. With up to a dozen options locally, the differences between CarePlus plans may matter more than the difference between carriers.

5. Enroll between October 15 and December 7, 2026 for coverage starting January 1, 2027 — unless a Special Enrollment Period applies to you.

CarePlus Health Plans FAQs

➤  Is CarePlus the same as Humana?

CarePlus Health Plans is a wholly owned subsidiary of Humana, but it is not the same thing as a Humana plan. CarePlus operates under its own name, its own Medicare contract and its own provider network, and it sells only in Florida. If you are enrolled in CarePlus, you are a CarePlus member rather than a Humana member.

➤  What states does CarePlus cover?

Florida only, and only part of it — roughly 20 counties across South Florida, Central Florida, West Florida, North Florida and the Atlantic Coast. CarePlus has never sold Medicare Advantage outside Florida.

➤  Are all CarePlus plans $0 premium?

Every CarePlus Medicare Advantage plan has carried a $0 monthly premium, including its special needs plans. That covers the plan premium only. You still pay your Medicare Part B premium, though the CareFree Giveback plans reduce that amount by an sum that varies by plan and county.

➤  Does CarePlus offer a PPO?

No. Every CarePlus plan is HMO-based. Some are HMO-POS, which adds a point-of-service option giving limited out-of-network coverage, but CarePlus does not sell a true PPO. If you need full out-of-network flexibility, look at a carrier that does.

➤  What is the difference between CareNeeds and CareBreeze?

CareNeeds plans are dual-eligible special needs plans for people who have both Medicare and Florida Medicaid. CareBreeze is a chronic condition special needs plan built around chronic lung conditions including COPD. Both require you to qualify — Medicaid eligibility for CareNeeds, a qualifying diagnosis for CareBreeze.

➤  Will CarePlus plans change in 2027?

Plan names, county availability, copays and supplemental benefits can change every year for any carrier. Your Annual Notice of Change, mailed by September 30, lists exactly what changes for your specific plan. If your plan is ending, you may qualify for a Special Enrollment Period that runs past December 7.

New to Medicare?

If you are new to Medicare, then you can enroll in a 2027 Medicare Advantage plan up to three months prior to your Medicare Part B effective date.
