SummaCare Medicare Advantage 2027: The Six Gemstone Plans and Where Each One Is Sold

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated November 3, 2026

SummaCare, the Akron health plan affiliated with Summa Health, files six individual Medicare Advantage plans for 2027, all HMO or HMO-POS, in 33 Ohio counties. They share one CMS contract, H3660, and SummaCare says its Medicare Advantage plans earned 4 out of 5 stars from CMS for 2027.

The plans carry gemstone names (Emerald, Sapphire, Ruby, Topaz, Quartz and Amber), but they are not sold everywhere SummaCare operates. Our count of CMS’s 2027 plan-county files shows that only 15 northeast Ohio counties get all six; in parts of northwest and west-central Ohio you may see just two. This guide shows which plans reach your county and how SummaCare stacks up against the national insurers selling in the same places.

Emerald and Sapphire are HMO-POS plans, which add some coverage outside the SummaCare network at a higher cost share. The other four are standard HMOs, so outside emergencies and urgent care you generally need SummaCare network providers.

Our Analysis: How Many SummaCare Plans Reach Your County

Our analysis of CMS 2027 plan data

✔

All 6 plans (15 counties): Ashtabula, Carroll, Columbiana, Cuyahoga, Geauga, Lake, Lorain, Mahoning, Medina, Portage, Stark, Summit, Trumbull, Tuscarawas and Wayne.

✔

5 plans: Ashland, Fulton, Holmes, Huron, Lucas, Seneca and Wood. 4 plans: Ottawa. 3 plans: Allen, Hancock and Putnam.

✔

Only 2 plans: Auglaize, Defiance, Erie, Henry, Mercer, Sandusky and Van Wert.

✔

No PPO, no special needs plan, no “Giveback” plan: all six SummaCare plans are HMO or HMO-POS plans open to anyone with Medicare A and B in the service area.

✔

Summit County (Akron): 96 individual plans from 17 insurer brands; SummaCare files 6, Humana 18 and UnitedHealthcare 12.

That makes SummaCare a regional choice, not a statewide one. Ohio has 176 individual Medicare Advantage plans for 2027 by our count, and SummaCare’s six are about 3% of them. See every insurer and county in our Ohio Medicare Advantage guide.

A Closer Look: SummaCare Medicare Ruby (HMO) for 2027

SummaCare’s 2027 Summary of Benefits for Ruby gives a sense of how its premium plans are built:

Ruby (HMO), 2027 In network
Monthly plan premium $55, plus your Part B premium
Medical deductible $0
Out-of-pocket maximum $4,200 (not including drugs)
Primary care / specialist $0 / $35 copay
Inpatient hospital $260 a day for days 1 to 6, then $0
Part D deductible $200, for drugs on tiers 3, 4 and 5
Dental $2,000 calendar-year maximum, preventive and comprehensive
Eyewear / OTC $250 a year / $30 a quarter

Source: SummaCare 2027 Ruby (HMO) Summary of Benefits.

A $4,200 in-network cap is on the low side for Medicare Advantage, but it comes with a monthly premium. The other gemstone plans have their own premiums and copays, so read each plan’s 2027 Summary of Benefits rather than assuming they match Ruby.

One of SummaCare’s 2027 plans is called SummaCare Medicare Quartz (HMO). It has nothing to do with Quartz Medicare Advantage, a separate insurer that files 19 HMO plans for 2027 in Wisconsin, Iowa and Minnesota, according to the same CMS files. If you are in Ohio and searching “Quartz Medicare,” make sure the plan ID begins with H3660, SummaCare’s contract.

The same caution applies to plan names in general. “Ruby,” “Topaz” and “Amber” are SummaCare product names; they do not tell you the benefit level. Compare the actual premium, out-of-pocket maximum and drug tiers side by side.

SummaCare's 2027 Star Rating

SummaCare’s quality ratings page says its Medicare Advantage plans “earned a 4 out of 5 stars for 2027” from CMS. In 2022, SummaCare announced that its 2023 Medicare Advantage plan had earned a 5-star rating, so 2027’s 4 stars is a step down from that peak.

Four stars still meets the level Medicare uses for quality bonus payments, which plans often put toward extra benefits. It does not give SummaCare the special year-round enrollment window that only 5-star plans get. Compare all carriers on our 2027 star ratings page.

Choosing between local and national plans?

In Summit, Stark, Cuyahoga and Mahoning counties, SummaCare competes with 15 or more other insurer brands. Local plans like SummaCare, Medical Mutual and PrimeTime Health Plan may have narrower, regional networks. Check that your doctors and hospital are in the 2027 directory before you enroll.

How to Check a SummaCare Plan for 2027

1

Count the plans in your county using the list above. In Erie, Sandusky or Van Wert, for example, only two SummaCare plans are filed.

2

Search the 2027 provider directory for your doctors and hospital under the exact plan name.

3

Price your prescriptions on SummaCare’s 2027 formulary. Part D out-of-pocket costs are capped at $2,400 for the year on every plan.

4

Compare the out-of-pocket maximum, not just the premium. A $0-premium plan with a higher cap can cost more in a year with a hospital stay.

5

Enroll by December 7, 2026 for coverage that starts January 1, 2027.
