How the Devoted Choice Giveback Part B reduction works: $174.90 a month back versus deductible, copays and out-of-pocket maximum

How the Part B Giveback Works

A Part B giveback is a reduction in the Medicare Part B premium you would otherwise pay. In Devoted’s words for this plan: “If you are responsible for a Part B premium, we will reduce your monthly Part B premium by $174.90 per month.” Over a full year, that is up to $2,098.80.

The reduction usually shows up as a smaller deduction from your Social Security check, or a smaller Part B bill if you pay Medicare directly. It can take a few months to appear after your coverage starts, depending on how Social Security processes the change.

Who does not get the giveback

Devoted’s Summary of Benefits states that if your Part B premium is paid by Medicaid or others on your behalf, you are not eligible for the $174.90 reduction. If you receive Medicaid or Extra Help, your cost sharing may also be lower than the amounts listed on this page.

The trade-off is how the plan is funded. Giveback plans generally put more of the cost on the moments you use care, so the right way to judge one is to add the giveback you receive against the copays, deductibles and out-of-pocket limit you could face. For more on how giveback plans work in general, see our guide to the 2027 Medicare Part B premium.

What You Pay for Medical Care

Because this is a PPO, it covers care from providers outside Devoted’s network as well as inside it. Some costs are the same either way, and some are higher out of network.

Service In network Out of network
Primary care visit $0 $5
Specialist visit $50 $50
Inpatient hospital stay $490 a day, days 1 to 5; $0 from day 6 $490 a day, days 1 to 5; $0 from day 6
Emergency care $115 a visit, waived if admitted within 24 hours $115 a visit
Urgent care center $40 $40
Outpatient mental health $50 $50
Skilled nursing facility $0 a day, days 1 to 20; $221 a day, days 21 to 100 See Evidence of Coverage
Ground ambulance $350 per one-way trip $350 per one-way trip

The number that matters most in a bad year is the out-of-pocket maximum. On this plan it is $9,850 for in-network care and $14,500 for in- and out-of-network care combined. The $9,850 figure is the highest in-network limit CMS allows for 2027, which is part of how the plan funds the giveback.

Prescription Drug Coverage

Unlike some giveback plans, this one includes Part D drug coverage. There is a $461 pharmacy deductible that applies to Tiers 3 through 5. At a network retail pharmacy, a 30-day supply costs:

✔

Tier 1, preferred generic: $0

✔

Tier 2, generic: $0

✔

Tier 3, preferred brand: 15% of the total cost

✔

Tier 4, non-preferred drug: 25% of the total cost

✔

Tier 5, specialty: 26% of the total cost

Insulin costs no more than $35 for a 30-day supply. The federal Part D out-of-pocket cap for 2027 is $2,400, after which covered drugs cost nothing more that year. If you take brand-name drugs, check each one on the plan’s 2027 formulary before deciding, because the percentage tiers can add up quickly.

Dental, Vision, Hearing and Other Extras

According to Devoted’s 2027 Summary of Benefits for this plan:

✔

Dental: a $250 yearly allowance for preventive and comprehensive dental. You can see any licensed U.S. dentist, pay first and then request reimbursement. Cosmetic procedures, implants and elective procedures are not covered.

✔

Vision: a $0 routine eye exam once a year and up to $200 a year toward eyeglasses or contacts.

✔

Hearing: a $0 in-network routine hearing exam once a year, and TruHearing Advanced or Premium hearing aids at $599 or $899 per aid, up to two aids.

✔

Fitness: a $0 SilverSneakers membership at participating locations, plus $150 a year in Devoted Health Wellness Bucks for fitness and wellness items and activities.

✔

Devoted Dollars: a $50 grocery and gas reward card for completing your yearly check-up with your primary care provider, if you opt in.

✔

Worldwide emergency coverage as a supplemental benefit.

Routine transportation is not covered on this plan. Benefits on other Choice Giveback versions, and on other Devoted plans, differ.

Who a Giveback PPO Like This Tends to Suit

Tends to suit: people who pay their own Part B premium, use relatively little care, take mainly generic drugs, and want the flexibility of a PPO network. For them, $174.90 a month back can outweigh the higher cost sharing.

Tends not to suit: people whose Part B premium is paid by Medicaid or others, who are not eligible for the reduction; people with frequent specialist visits or planned hospital stays; and people who rely on brand-name drugs in Tiers 3 to 5.

A simple way to compare: take the yearly giveback you would receive, then subtract a realistic estimate of the deductible, copays and drug costs you expect. Do the same for any other plan you are considering. The plan that leaves you better off in a normal year and a bad year is the one to look at more closely.

Star Rating and Devoted Health

Star ratings attach to a carrier’s contract rather than to a plan name. In the 2027 star ratings released October 8, 2026, Devoted’s contracts ranged from 3 to 4.5 stars depending on the state, and Devoted was among the carriers that increased the share of members in plans rated 4 stars or higher. Newer contracts can show “plan too new to be measured.” Check the rating shown for this plan in Medicare Plan Finder.

For the national picture, see the 2027 Medicare Advantage star ratings.
