Does Medicare Advantage Cover You Out of State? Travel Rules for 2027

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated November 7, 2026

Medicare Advantage plans are local. Each one is approved for a service area of specific counties, and its network is built around the doctors and hospitals there. That works well until you visit grandchildren in another state, spend winters in Arizona or Florida, or take a long road trip.

This guide explains what every Medicare Advantage plan must cover away from home, where the plan types differ, and the residency rule that catches snowbirds. It also uses our count of CMS’s 2027 plan files to show how many plans give you some out-of-network coverage, and which companies sell PPOs in the most states.

Medicare Advantage out-of-state coverage 2027: 1,389 PPO and 1,316 HMO individual plans; 2,938 of 3,018 counties have a PPO or PFFS plan; plans usually allow 6 months away

What Every Medicare Advantage Plan Covers Away From Home

Whatever plan type you have, these are covered anywhere in the U.S.:

✔

Emergency care. If you reasonably believe your health is in serious danger, go to the nearest emergency room. The plan cannot require prior approval or charge more because the hospital is out of network.

✔

Urgently needed care when you are temporarily outside the service area, such as an infection or a sprain that cannot wait until you get home.

✔

Kidney dialysis when you are temporarily out of the service area, at in-network cost sharing.

What is not guaranteed is routine care: a checkup, a specialist visit, physical therapy or a planned surgery in another state. Whether those are covered depends on the plan type and the plan’s own travel rules.

HMO vs. PPO vs. PFFS When You Travel

Plan type Routine care out of state 2027 individual plans (non-SNP)
HMO Generally not covered except emergencies, urgent care and dialysis 1,316
HMO-POS Some out-of-network services may be covered, usually at a higher cost; rules vary by plan 785
PPO Covered out of network at a higher cost share, if the provider agrees to see you 1,389
PFFS Any Medicare provider who accepts the plan’s payment terms 11
MSA Any Medicare provider; you pay from a deposit until a high deductible is met 1

A PPO is the usual choice for people who travel, but out-of-network coverage is not the same as in-network coverage. Out-of-network costs count toward a higher combined out-of-pocket maximum, and a provider out of state may decline to bill your plan. Call ahead before a planned visit.

Our Analysis: PPO Access in CMS's 2027 Files

Our analysis of CMS 2027 plan data

✔

2,938 of 3,018 counties with individual Medicare Advantage plans have at least one PPO or PFFS plan for 2027. The 80 without are mostly in California (32) and Texas (23).

✔

Riverside County, California lists 46 individual non-SNP plans for 2027, but only 4 are PPOs; 35 are HMOs.

✔

Snowbird counties have PPO choices: Maricopa, AZ has 14 PPOs among 50 plans; Lee, FL 14 of 29; Palm Beach, FL 16 of 44; Clark, NV 12 of 38.

✔

Humana sells PPOs in 46 states, UnitedHealthcare in 43, Aetna in 40 and Devoted in 31, which matters if you want the same company’s network in two places.

Selling PPOs in many states does not mean one nationwide network. Each PPO plan still has its own service area, and its in-network benefits apply to that plan’s network.

Snowbirds: The 6-Month Rule and Visitor/Traveler Programs

Medicare Advantage plans generally must disenroll you if you are away from the service area for more than six months in a row. Medica, for example, lists being “away from the plan’s service area for more than six months” as a reason for disenrollment. In September 2026, 24/7 Wall St. described a Minnesota retiree who left for Arizona on November 1 without a traveler benefit and whose plan would drop her on June 1.

Some plans offer a visitor/traveler program that lets you stay enrolled for up to 12 months in certain areas. HMSA in Hawaii, for example, says members may remain in the plan out of the service area for up to 12 months where it offers the program, and pay the same in-network costs through participating Blue Medicare Advantage PPO providers. Other Blue Cross plans have similar arrangements, but participation varies, so check your plan’s Evidence of Coverage.

Two homes? Pick the right address

Your plan is tied to your permanent residence. If you split the year evenly, choose a plan where you live most of the year and confirm how it treats the other state. If you move for good, you get a special enrollment period to choose a plan in your new area.

Outside the U.S.

Original Medicare generally does not cover care outside the United States. Some Medicare Advantage plans add worldwide emergency or urgent care coverage as an extra benefit, often with a yearly dollar limit. Look for “worldwide emergency” in the plan’s Summary of Benefits, and consider travel medical insurance for longer trips.

How to Choose a Plan If You Travel

1

Count your days away. More than six months in a row usually rules out a standard local plan.

2

Prefer a PPO if you want routine care in another state; check the combined out-of-network maximum.

3

Ask about a visitor/traveler program and which states it covers.

4

Check pharmacy access in the other state; most plans have national pharmacy networks, but preferred pharmacies vary.

5

Look for worldwide emergency coverage if you travel abroad.

6

Compare plans by county in our Medicare Advantage PPO guide and enroll by December 7, 2026.
