UCare Medicare Advantage in 2027: The Plans Are Gone. Here Is Who Sells in Minnesota Now

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated November 3, 2026

There is no UCare Medicare Advantage plan for 2027. UCare closed all of its Medicare Advantage plans at the end of 2025, and our search of CMS’s 2027 plan files finds no plan filed under the UCare name for next year. UCare itself said its Medicare Advantage plans “are closing and are not part of the agreement with Medica,” the insurer that took over other UCare business.

If you were a UCare member and landed in a plan for 2026 in a hurry, the 2027 Annual Enrollment Period (October 15 to December 7, 2026) is your first full chance to shop on your own schedule. This guide explains what happened to each UCare Medicare product and uses our own count of CMS 2027 data to show which insurers now sell Medicare Advantage in each part of Minnesota.

MPR News reported that UCare’s Medicare Advantage exit affected about 158,000 people, and that UCare, which served more than 300,000 members across Minnesota and western Wisconsin, was shutting down with its remaining plans moving to Medica. Medica did not take over the Medicare Advantage plans; former members had to choose a new plan themselves.

Our Analysis: Minnesota Medicare Advantage After UCare

Our analysis of CMS 2027 plan data

✔

53 individual plans from 11 insurer brands are filed for 2027 across all 87 Minnesota counties. 40 of them (75%) are PPOs.

✔

Medica has the widest reach: 12 plans filed in 71 counties, including Medica DUAL Solution (HMO D-SNP) in 54. Blue Plus’s SecureBlue D-SNP reaches 55 counties and Blue Cross and Blue Shield of Minnesota’s own plans 35.

✔

No UCare, no EssentiaCare, no Humana. The 2027 files list no plan under the UCare or Essentia names and no individual Humana plan in Minnesota.

✔

39 of 87 counties have only one or two insurer brands selling Medicare Advantage. Aitkin, Carlton, Cook, Crow Wing, Koochiching and Lake counties each have just 2 plans.

✔

Twin Cities vs. Duluth: Hennepin County has 25 plans from 6 brands; St. Louis County has 6 (UnitedHealthcare 4, Blue Plus 1, Medica 1).

The gap between the metro area and the north is the main thing former UCare members should know. UCare’s EssentiaCare plan was built with Essentia Health, which is based in Duluth; for 2027, people in much of northern Minnesota are choosing among a handful of plans. See every county in our Minnesota Medicare Advantage guide.

If You Had MSHO or Connect + Medicare Through UCare

Minnesota’s dual-eligible programs pair Medicare with Medical Assistance (Minnesota Medicaid) in one plan. For 2027, the CMS files list these dual special needs plans (D-SNPs) in Minnesota:

2027 dual plan Insurer Counties filed
SecureBlue (HMO D-SNP) Blue Plus 55
Medica DUAL Solution (HMO D-SNP) Medica 54
PrimeWest Senior Health Complete and Prime Health Complete (HMO D-SNP) PrimeWest Health 24
HealthPartners Minnesota Senior Health Options (HMO D-SNP) HealthPartners 12
SeniorCare Complete and AbilityCare (HMO D-SNP) South Country Health Alliance 7
IMCare Classic (HMO D-SNP) Itasca Medical Care 1 (Itasca)

Which ones you can join depends on your county and your Medical Assistance eligibility. People with both Medicare and Medicaid can usually change plans more often than once a year, so check with your county or the plan about timing.

Who Sells Medicare Advantage Where in Minnesota for 2027

Plan counts by insurer only tell part of the story in Minnesota, because several insurers sell in a few metro counties while others cover most of the state. Here is each insurer’s 2027 reach by county:

✔

Statewide or nearly so: Medica (71 counties) and Blue Plus (55, dual plan only).

✔

Regional: Blue Cross and Blue Shield of Minnesota (35 counties), PrimeWest Health (24) and Align powered by Sanford Health Plan (19).

✔

Metro-focused: UnitedHealthcare, Allina Health Aetna Medicare and HealthPartners (15 counties each).

✔

Humana is not on the list. The CMS 2027 files show no individual Humana Medicare Advantage plan in Minnesota; read more on our Humana plans ending in 2027 page.

Moved to a plan in a rush last winter?

Former UCare members had until February 28, 2026 to join a new Medicare Advantage plan. If you took the first plan you found, compare it now: Annual Enrollment runs October 15 to December 7, 2026, and a new plan starts January 1, 2027.

How Former UCare Members Can Shop for 2027

1

Start with your county. In the north, there may be only two or three plans; in the Twin Cities, there may be 25 or more.

2

Check your clinic and hospital first, especially if you used Essentia Health, M Health Fairview or a regional system. Search the 2027 directory for each plan you consider.

3

Price your drugs on the 2027 formulary. Part D out-of-pocket costs are capped at $2,400 for 2027.

4

If you have Medical Assistance, compare the dual plans in the table above rather than a standard plan.

5

Consider Original Medicare plus a supplement if no plan in your county includes your doctors; ask a licensed agent about Minnesota’s supplement rules before you drop Medicare Advantage.
